Job Title: SERVICE COORDINATOR I - Bilingual Required
Job Summary: Entry level Service Coordinator, who within a team environment, provides consultation, advocacy, resource information and service coordination for people with developmental disabilities who require less involved and specialized assistance.
Scope of Individuals to be Served:
Individuals who are the most stable between the ages of 3 and 15 that are already attending school, a day program, or are in supported employment.
Specific Job Duties:
1. Support individuals by maintaining regular contact and doing Individual Program Plans (IPP).
2. Participate in Interdisciplinary Team (IDT) staffings and attend meetings in the community, as needed, including Individualized Education Plans (IEP) and day program meetings.
3. Information and referral to generic services and resources in the community, including but not limited to:
a. Schools, recreational options and local support groups.
b. Supportive Employment/Habilitation funded day programs.
c. Some local agencies (Social Security, Medi-Cal, IHSS).
d. Specialist physicians/ specialists (speech therapist, O.T./P.T.).
4. Coordinate and secure TCRC funded services, including in-home and out-of-home respite, durable medical equipment and specialized services, and non-Medi-Cal funded services.
5. In addition to the case management responsibilities SC-1 job duties include:
a. Arranging periodic meetings at least one time per year, within the team to ensure smooth individual transition from Early Start (ages 0-3) to Children’s Team (ages 3-22) and from Children’s Team to Adult Team (ages 22-death).
b. Tracking case assignments with manager.
c. Providing team support by performing additional team functions as agreed by each individual team and branch manager.
6. Attendance at community meetings.
7. Complete the annual Medi-caid Waiver review form annually on individuals we serve who are living with their families and have less intense clinical needs. This MW form would be completed along with the usual annual review documentation. (The Individual Service
Manager will serve as the QMRP for all MediCaid waiver individuals served by an SCI and must sign the document.)
8. SC 1’s will not participate in the Officer of the Day rotation.

11. Develop and maintain good relationships with community representatives.
12. Participate in professional growth and development through attendance at in-service and other
training activities, conferences and other job-related agency approved events.
13. Conduct all activities in a professional and ethical manner.
14. Additional or different functions may be assigned from time to time.

Minimum Qualifications:
 Bachelor’s degree in a Human Services or related field.
 Valid California drivers license and reliable, insured transportation

Physical Requirements:
Adequate manual dexterity and coordination for operation of an automobile, computer, and other
standard office equipment is required for this position. The ability to sit at a workstation for long periods
of time, the ability to read, write and comprehend large amounts of written material such as reports and
regulations, use the telephone, and communicate via electronic mail are all required on a daily basis. The use of an automobile and the ability to drive is absolutely required on a daily basis. The ability to stand,
sit or walk for moderate distances on uneven terrain is also required on a daily basis. While performing the duties of this job, the employee is regularly required to use hands to finger, handle, feel or operate objects, tools, or control and reach with hands and arms. The employee frequently is required to stand, sit, walk, push, pull, kneel, stoop, bend, and squat. The employee must
frequently lift and/or move up to 10 lbs and infrequently up to 20 lbs. Specific vision abilities required by this job include close vision, distance vision, color vision, depth perception, and the ability to adjust
focus. Each of these physical tasks is an essential function of the position.
