Position Purpose:

The Community Health Worker-COVID Outreach Initiative, is a temporary position that is being deployed to conduct COVID-19 outreach and education to communities most impacted by the virus.

Position Responsibilities and Functions:

  • Responsible for advancing the goals of the CHW Outreach Initiative, which include the following:
  • Amplify existing COVID-19 messaging and provide general education outreach including
  • What COVID-19 is, how it can spread, what to do when you have it, and how individuals and families can stay safe;
  • How to access services including, but not limited to, social services, health insurance coverage, medical and behavioral health services, and availability of COVID-19 prevention services (e.g., testing, contact tracing);
  • Current directives, what’s open and not open, employee/employer requirements, and worker rights;
  • How to respond if DPH calls you for contact tracing, the importance of contact tracing; and
  • Dispelling myths and conducting rumor control, including where to find accurate information and updates.
  • Maintain outreach activity calendars and logs according to program standards.
  • Documents work with clients through appropriate record keeping that follows East Valley’s policies and procedures.
  • Links clients to needed services and facilitates access to community resources.
  • Advises clients and others regarding health care and other facilities available to them; assists patients in utilizing services; makes follow up contact when required.
  • Attend regularly-scheduled and impromptu meetings and maintain communication with program team members and supervisor.
  • Work in collaboration with other departments and agencies.
  • Responsible for complying with HIPPA standards and observing strict patient confidentiality.
  • Other job duties and responsibilities as assigned.

Position Requirements and Qualification:
  • Knowledge of community resources in area of residence.
  • Comfortable working with diverse populations.
  • Exceptional ability to connect and engage with people.
  • Good oral skills.
  • Able to work flexible job hours.
  • Bilingual English/Spanish preferred (read, write, speak)
